If you intentionally submit the independent healthcare virtual-assistant referral form, the contact and practice information entered there is transmitted directly to the independent provider identified on that page so the provider can respond to your request. It is not added to OneSource OS or OneSource's ordinary sales intake. The form must not be used to send PHI, patient-specific details, credentials, or confidential documents.
We may share information with trusted service providers solely to support business operations, such as:
